Two location Medicare and Medicaid Certified home health agency serving San Francisco, San Mateo, Santa Clara, Napa, Solano, Yolo and Contra Costa counties in the San Francisco Bay Area with comprehensive skilled nursing, PT, OT, ST, MSW and Home Health Aid services along with other specialized clinical offerings. Current census of 193 with roughly 70 new admissions per month across each location. Idaho: $3M Home Care Agency

This opportunity features a non-medical in-home care agency operating primarily in Northern Idaho. Founded in July 2013, the agency provides assistance with daily living tasks—including personal care, meal preparation, housekeeping, companionship, and transition management—to the elderly and disabled who would otherwise require facility care. The business operates from two locations, maintaining a main office in Shoshone County and a satellite suite in Kootenai County. Currently, the agency serves a census of 178 clients across four counties. The agency is fully credentialed with Idaho Medicaid and major managed care organizations (MCOs). Maryland:  $1.2M Home Health and Private Duty  Agency (SOLD)

This agency is located in Maryland and operates from one location within a senior living community since 2012. The agency is Medicare Certified and state accredited (with RSA License) and provides skilled care along with a private duty/private pay service line which accounted for over 99% of the $1.2 million of gross revenue in 2024. Medicare Certificate of Need (CON) is limited to certain senior living communities in the areas they serve. Key employees are available for continued employment as needed. Massachusetts: $5.4 M Medicare/Medicaid Home Health Agency

Founded in 2006 and Medicare/Medicaid certified since early 2007, this clinician-owned, full-service home health agency operates across the North Shore, South Shore, and Cape Cod regions of the greater Boston market. The company provides skilled nursing, wound care management, medication administration, psychiatric nursing, and physical therapy services. It operates from two leased office locations in eastern Massachusetts and primarily serves an elderly and adult psychiatric patient population. The agency differentiates itself by accepting medically complex patients, including those with significant psychiatric and multiple comorbid conditions, that many competing agencies decline. The business holds 11 active payer contracts, including Medicare, Medicaid, and multiple Massachusetts MCO/ACO plans. North Carolina: Private Pay Home Care Agency (SOLD)

The agency has been in operation since 2011 and provides non-medical services to seniors.  The agency is private pay and licensed  by the state to provide services to the counties of Craven, Lenoir, Carteret, Pamlico, Pitt, Jones, Beaufort, Greene, Onslow and Pender. They employ 28 PT/FT W-2 caregivers along with an RN supervisor, director and two administrative personnel.  It is expected the staff will continue following a sale with the exception of the director who is also the owner.  Excellent demographics in the area  for continued growth.  2024 Gross Revenues of $797,228 with Seller Discretionary Earnings of $186,208.

Texas:  Hospice Agency (SOLD)

Medicare, Medicaid certified and CHAP accredited agency located in Region 8 and operation since 2022. Medicaid license allows for service to all 11 regions of the state. Expansion of regions for Medicare can be added via application process with the Texas Health and Human Service Commission. PRN staff is available for continued employment along with admin staff if needed. This is a small census agency so value is the license, certifications, contracts and accreditation.  Value of business is the license, accreditation and contracts held.
